Help with Enrollment Form

Hey guys, 

I am looking for some help with a Google Form. I am wanting to create a Form programs can use for enrollment, where students would fill in their information using the computer. The idea is more fully explained in "A New Orientation."

Currently I have the form made, but I am wanting to use Google Sctipts to have the form send itself to the proctor. I am seeing it being a mail merge as described in this Google how-to article.

The goal is to have a form the new student fills, upon submittal it is e-mailed to the proctor, the proctor prints the form, the student signs the printed form, and the form is then placed in the student folder.

I could use any help offered. Please feel free to forward this post with anyone you think may be interested.
